The Short Answer
PIP for a mental health condition is assessed on exactly the same terms as any other claim. Nine of the twelve activities can score on mental health grounds, and two of them are described in the DWP's own guidance as designed for limitations deriving from mental health, cognitive and learning impairments. Both components can reach the enhanced rate. What decides a claim is the descriptors, the frequency, and evidence that describes function rather than diagnosis.
Which Activities Score
Nine of twelve, and only moving around is effectively closed. The DWP assessment guide states the design intent for two of them: engaging with other people "is designed for limitations on social engagement deriving from mental health, cognitive and learning impairments", and planning and following journeys "is designed for limitations on mobility deriving from mental health, cognitive and sensory impairments".

That distribution is the single most useful fact in a mental health claim. A claim described only under the first two activities caps at 8 daily living points, and the remaining points sit in the seven below.
The Defined Terms That Decide the Top of the Scale
Two terms, both narrow, both worth quoting on a form.
Overwhelming psychological distress means, in the assessment guide's words, "distress related to a mental health condition or intellectual or cognitive impairment which results in a severe anxiety state in which the symptoms are so severe that the person is unable to function". It sets the bar for the 8-point engagement descriptor and for the 10-point journeys descriptor.
Social support means support from a person trained or experienced in helping with social engagement, and family or friends can provide it. The distinction from prompting is worth 2 points: prompting is being reminded or encouraged before an interaction, social support is needing someone present during it.

The same rates as any award, because there is no mental health rate.

Evidence That Works
Records of what happens, not records of what you have. The diagnosis is rarely the point in dispute; the frequency and reliability of the difficulty is.
- A care plan or crisis plan, naming the support provided and by whom.
- A support worker or family statement, describing what they do, how often, and what happens when they do not.
- A repeat prescription list, which answers the medication activity without recall.
- Records of crisis contacts or admissions, which establish severity across time rather than at a point.
- A dated log over four to six weeks, recording the days an activity did not happen. This is the document that answers the majority-of-days rule directly, and almost no clinical record does.
Send copies with the claim form and add anything later as it arrives. Evidence can be added after the form is returned and up to the decision.
Why Claims Are Refused
Three patterns account for most of it, and all three are avoidable.
The first is scope: describing the claim only under engagement and journeys, leaving seven scoring activities blank. A blank section is scored at zero, not held open.
The second is framing: describing distress rather than function. Every descriptor above zero already assumes the condition is present, so "I feel anxious about going out" scores nothing that "I have not made an unaccompanied journey since March" does not.
The third is frequency: describing the worst weeks, which invites the conclusion that the activity is possible on most days. The test is more than half the days, so the ordinary week is the accurate answer.
Challenging a Decision
One month from the decision letter to ask for a mandatory reconsideration. Of DWP decisions cleared at a tribunal hearing between October 2020 and September 2025, 65% were overturned in the claimant's favour.

The points section of the letter names the descriptor chosen for each activity, which locates the disagreement precisely. Where the mental-health-relevant activities were scored at zero without being addressed, that is the substance of a reconsideration.
